All About Open vs. Closed Rhinoplasty
There are two methods of rhinoplasty; close method that involves internal incisions to adjust the structure of the nose and open method that includes a second incision in the tissue between the nostrils called the columella.
A Rhinoplasty procedure can take up to six hours to perform. The length of surgery largely depends on the grafting needing to be completed as well as the work required to achieve identified outcomes and patient requests. Usually general anesthesia is provided during the procedure. Patients can expect longer surgery and recovery times for procedures that require opening up the nasal passage for unobstructed breathing or the like.
The Closed Rhinoplasty, also known as the Endonasal Rhinoplasty, involves minimal surgical incisions to the outside of the nose. It is typically performed with all surgical incisions made inside the nostrils; meaning that the incisions will not be seen externally. The surgeon navigates under the skin over the bridge of the nose. The entire procedure is done without direct visual contact to the nasal structure and passage but rather the use of feel.
Overall the method is done with limited visibility. The best way to grasp Closed Rhinoplasty is to imagine molding and shaping an expensive piece of pottery under a large sheet. The limited visibility can sometimes hamper the surgeon’s ability to achieve the desired results of the patient. Often the patient will experience a high degree of distorted nasal cartilage.
Some of the benefits of a Closed Rhinoplasty include a decrease in possible scarring including, no columellar scarring and decreased postoperative edema. There is also the added benefit of a decreased opportunity for the need for nasal tip support reduction.
The Open Rhinoplasty, better known as the External Rhinoplasty, is the somewhat opposite of the Endonasal Rhinoplasty. This approach involves external incisions. An incision is made across the bridge of the nose, known as the trans-columellar incision. It connects the right and left incisions across the septum (the part of the nose that separates the nostrils). The nasal skin is then folded back and upward; providing full access to the nasal structure and passage.
An Open Rhinoplasty provides easier and greatly improved surgical access to the nasal passage and structure. Many surgeons liken this approach to the procedure to working under the hood of a vehicle. The open access enables a more precise approach for the reconstruction and/or modification of the structure of the nose.
Some of the benefits of the an Open Rhinoplasty include full access to the nasal passage, reduced cartilage tearing, immediate access to the bridge for shaping and restructuring and directly addressing nasal passage airways.
