Consent Forms

When you’ve decided to go ahead and undergo a surgical procedure after the initial consultation at TCC, you’ll be asked to fill out consent forms. These consent forms are procedure-specific, outlining details and instructions on the surgery itself as well as pre and post operative care. It is important that you take the time to go through each point thoroughly before signing off on it. If there is anything you odn’t understand, one of our helpful receptionists, the patient consultant, or a physician can explain and clarify the point for you.
Some of the things included in the consent forms are whether you were fully informed of the procedure you hope to undergo, authorization of the physician to perform the surgery and questions about your medical history. Completing the consent forms usually takes at least 15 minutes, as it is comprised of several pages.
You may think that it is troublesome and time consuming, but remember - a vital part of any surgical process is making sure you understand the procedure and are physically and mentally prepared for it. This is your body, and it’s absolutely imperative that you know what will be done to it, by whom, and what you can do to assist in proper preparation and recovery.
